Patients need to collect an application form from reception team or download via our Website.

I.D. will be required to verify your application at the time of returning your form to us & access to your record is at the GP’s clinical discretion. You will be contacted at a later date with this decision.

Summary Care Record - (medications, allergies and adverse reactions)

Achieved.

All patients registered at Helix Medical Centre have a Summary Care Record created unless an ‘opt out’ form is received.

Summary Care records can be accessed by GP’s, out of hours, A&E when patients attend, to facilitate safer prescribing outside of the practice.

GP2GP transfer (electronic transfer of computerised medical records between the patients registered GP)

Electronic, patient records are automatically transferred to the patients registered GP if available at their surgery & dependant on the record size.
