EMPOWER courses are structured education of Type 2 diabetes. The latest dates for courses and patients can book directly through their site. As more dates are available, they will be added to EMPOWER's site.

The National Diabetes Prevention Programme (NDPP)
The Practice is participating in the National Diabetes Prevention Programme (NDPP) which offers patients at risk of diabetes the chance to attend a behaviour change programme. This programme is being delivered on behalf of the NHS by 'Living Well Taking Control' (LWTC) to help people reduce their risk of diabetes.
Those referred will receive tailored, personalised help to reduce their risk of Type 2 diabetes including education on healthy eating and lifestyle, help to lose weight and bespoke physical exercise programmes, all of which together have been proven to reduce the risk of developing the disease.
You can contact your GP to be referred, the NDPP service no longer accepts self referrals.
Patients can self-register onto the programme, however patients must have had a recent blood glucose result and date to enable the form to be filled in correctly: Register | diabetes-prevention (lwtcsupport.co.uk)

Reading Well – Long term conditions
Hertfordshire Libraries now have a new collection of titles to support people with long term conditions. Reading Well provides accredited and helpful reading to support health and wellbeing. The books are selected by health professionals following a rigorous and evidence-based approach. The scheme has been co-produced with people who long term conditions.
The new booklist of 28 titles provides adults living with long term conditions with advice, information and support.
Specific conditions addressed are:
- arthritis
- bowel conditions
- breathing difficulties
- diabetes
- heart disease
- stroke
Some common symptoms covered include pain, sleeping problems and mental health and wellbeing. General information and advice on living well with a long term condition and support for family and carers is also included.
Titles can be reserved free of charge to pick up from any local library. Booklists are available in all libraries.
