Hertswise Dementia Community Support Service
The new Hertswise groups are for cared for and carers to attend, held every week in all areas of Hertfordshire and accessible to people with concerns about memory loss, people with memory loss but without a diagnosis and to people with a diagnosis of memory loss. The groups are led by a locality worker and activities are co-produced with the people who attend but all focus on improving physical, psychological and social wellbeing. Carer support and 1 to 1 support is also available, as well as information and advice on a range of issues including paying for care, accessing benefit entitlements, setting up Lasting Powers of Attorney, help with getting online and much, much more.
Dementia Support Service
This is a specialist dementia advice service for carers and people with dementia. It supports every person diagnosed with dementia by the Early Memory and Diagnosis Support Service (EMDASS) and can provide ongoing support for those who need it.
Admiral Nurses
The Specialist Dementia Crisis Support service is a complex mental health crisis function specifically designed for carers of people with dementia. It aims to reduce avoidable hospital admission due to carer breakdown. The nature of the complex issues causing an emerging crisis means that problems are best benefited by a nurse led case management approach, supporting the whole person and the cared for person with Dementia

New website: pre-memory clinic support - July 2021
DCAN (Dementia Change Action Network) have published the Next Steps website to support people who are waiting for a memory assessment appointment. The website is for anyone who has been referred for an appointment at a memory assessment service as well as family members, carers, and organisations who support them. It helps people to find the right support at the right time and includes information on what to expect from the assessment process and ways to support wellbeing.
